Advanced Primary Care Management (APCM)

Advanced Primary Care Management from CMS is designed to create a system of on going, team based, proactive patient care. Patients enrolled will have access to additional resources like 24/7 access to the provider’s team, coordinating care within a week of a hospital discharge, home visits, expanded hours and secure messaging options. On paper this sounds like a step in the right direction for managing the Medicare population more effectively by closing care gaps. For example, Cadence is using its Proactive Care Engine to identify gaps within the patients EHR. Cadence then takes action on those gaps by routing them to their medical group that operates 24 hours a day.
